Scholarship application deadline is Aug. 30, 2007.
Incomplete and/or faxed scholarship applications will not be accepted.
Minimum Requirements
- Must be a NANAINA member.
- Must be enrolled in a US Federal or state recognized tribe.
- Must be enrolled as a full-time undergraduate or graduate nursing student in an accredited or state-approved school of nursing.

Please download the file below, print it, fill it out, and mail back to the address provided under the files. If you are using Microsoft Word and would like to fill out the application on the computer, download the MS Word file, complete the form and email back to akoertvelyessy@hrsa.gov.
For the Membership Application
Please download, print, complete, and mail to address within the file. Thank you.
