Incomplete and/or faxed scholarship applications will not be accepted.

For the Merit Award Application

Please download the file below, print it, fill it out, and mail back to the address provided under the files. If you are using Microsoft Word and would like to fill out the application on the computer, download the MS Word file, complete the form.

For the Membership Application

Please download, print, complete, and mail to address within the file. Thank you.

Minimum Requirements

  • Must be a NANAINA member.
  • Must be enrolled in a US Federal or state recognized tribe.
  • Must be enrolled as a full-time undergraduate or graduate nursing student in an accredited or state-approved school of nursing.
